Successes

Community Paramedic Program

The Community Paramedic Program was created to bring skilled medical care directly to patients in their homes, focusing on people with complex health and social needs. Working in collaboration with local hospital partners, we led the development of the program’s proposal and design and secured start-up funding to launch the initiative.

In just the first quarter, the program reduced unnecessary emergency room visits by one-third among this group. By addressing underlying issues proactively, the program delivers compassionate, community-based care while helping to reduce strain on emergency services.

Sober Living Facility

Working in partnership with Langlade County leadership and local law enforcement, we helped propose and design a sober living facility specifically for women at high risk for recidivism. The initiative aimed to provide a safe, supportive environment that addresses the root causes of substance use and involvement with the justice system. The facility was developed as part of a broader effort to promote recovery, reduce repeat offenses, and support long-term stability for women in the community.

Mental Health Collaborative

We helped create the Mental Health Collaborative, a cross-sector partnership uniting schools, healthcare providers, law enforcement, and community organizations to address rising youth suicide rates. We helped lead efforts to define the collaborative's structure, align shared goals, and secure eight years of sustainable operational funding through a combination of grants and public-private partnerships. Together, we’ve built a more coordinated, responsive system of support for youth mental health.

Community Garden Program

We founded a community garden program in partnership with the local Federally Qualified Health Center (FQHC) to promote access to fresh, healthy food and strengthen community connections. Working with local partners, we transformed underused urban space into a shared garden where patients, staff, and neighbors could grow produce, learn about nutrition, and support one another. The program was designed to address food insecurity while fostering a sense of ownership and well-being among participants.

Crisis Assessment Response Teams

In partnership with local law enforcement, we designed and launched a new crisis response system known as the CART Team (Crisis Assessment Response Teams). This model pairs a trained crisis worker with a law enforcement officer to co-respond to mental health-related police calls in the community. The approach ensures individuals in crisis receive timely, appropriate care while reducing unnecessary arrests or emergency room visits. The program has led to more effective use of law enforcement resources and improved health outcomes for the individuals and communities it serves.
